The type of personal information we collect.

We need to keep comprehensive and accurate personal data about our patients in order to provide them with safe and appropriate dental care. We currently collect and process the following information:

Medical and dental records including Radiographs, clinical photographs and study models.

How we get the personal information and why we have it.

Most of the personal information we process is provided to us directly by you for example:

Your data will be collected via online forms, email, telephone calls, in person, or in referral letter or communication from other clinicians.

Data may also be collected if you sign up to marketing, social media, or other channels, this is only done for monitoring, and analytics, to gather preference, statistics to improve our service and marketing for similar visitors. We do not sell or disclose information to third parties.

We may receive information about you from your referring dentist in relation to your dental treatment.

We have and use the information that you have given us in order to plan and deliver your dental care. This may be in conjunction with or shared with other health professionals as required. We may disclose your information to individuals, organisations or third parties in order to provide proper care, this will only be done in line with other organisations who comply with applicable data protection legalisation and operate in the strictest confidence. In order to provide proper and safe dental care, we may need to disclose personal information about you to:

Other health professionals caring for you.

Dental Insurance Companies of which you are a member.

Private dental schemes of which you are a member.

Please note we will never sell your data or personal information to a third party.

If requested by law or by a court order, personal data may have to be disclosed to a third party not connected with your dental care. In all other situations, disclosure that is not covered by this policy will occur only when we have your specific consent.

How we store your personal information

Your information is securely stored. Personal data about you is held in the practice’s computer system and/or in a manual filing system. The information is not accessible to the public and only accessed by treating clinicians and relevant dental team members, who will only access the information necessary for your course of treatment or personal information for contact and communication.

We will retain your records while you are a practice patient. If you cease to be a patient, we will continue to hold them for at least another eleven years, or in the case of children until they reach the age of 25, whichever is the longer. We will then dispose your information by secure digital deletion software or physical destruction (shredding/other means of making unreadable) in the case of physical records.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
